To complete this form you will need to provide a copy of your proof of identity documents in the Residency and Citizenship section. You will need a digital copy of one of the following options ready to upload:
NZ Citizen – NZ Born – Driver Licence or Birth Certificate or NZ Passport
NZ Citizen – Overseas Born – NZ Passport or Birth Certificate AND Proof of Citizenship
NZ Resident/Permanent Resident – Passport AND Proof of Residency
Australian Citizen – Australian Passport
Australian Permanent Resident – Passport AND Proof of Residency
If you have changed your name by marriage, deed poll or statutory declaration, you will be asked to upload proof of your name change in the Applicant Details section.
If you have an Overseas Qualification, you will be asked to upload copies of your qualification and/or results in the Applicant History section.

All enrolment applications are subject to approval. MAST will acknowledge receipt of your application. Confirmation of enrolment and acceptance into the course will be within 30 days of receipt.
Campus based courses payment will be required prior to starting. Once enrolment is approved, payment can be submitted.
